To allow your Doctors or Medical Care Providers to upload your medical records directly to your PHR account you will need to create a Medical Care Provider Pass Code. Do this by logging in to your account and from the Welcome Page, click the button" Create Care Provider Pass Code". This Pass Code is separate from the Password you use to log in to your PHR account.

Care Provider Upload Option

Medical Care Providers will have restricted access to your PHR account. The Provider's access will be limited to specific functions, uploading of medical records, lab reports, x-rays and other medical related documents. The Provider will have the ability to add multiple files at one time, preview/add/remove and enter notes for each uploaded document prior to final submission to your account.

Each file uploaded by the Provider will be located in the My Medical Reports section of your account. You will receive an email notification each time a Provider adds a file to your account. We recommend that you review each new file and select to show or not show these files as part of your Emergency Summary Report which is available to Doctors or Emergency Responders by clicking on the red bar at the top of the Access My Records.com website.

Your Doctor or Care Provider will need the website address www.accessmyrecords.com, your Access ID Code, Last Name and the Care Provider Pass Code you created in order for him/her to upload records. We have provided you a label format that you can copy, paste and print, this has all the information your Doctor or Care Provider will need to upload medical files directly to your PHR account.
