The AccessID^SM Card contains the name and AccessID^SM code of the member. This card provides emergency service personnel the information needed to retrieve vital medical records and legal documents that will assist them during an emergency situation. Medical personnel may simply log into www.accessmyrecords.com, enter the members name and AccessID^SM code found on the member's AccessID^SM Card or they may call our 24/7 emergency hotline. They will be provided with a read-only summary of the member medical and legal records. Your medical information is otherwise completely private and secure.

How Do I Update My Medical Records?

You can enter your personal medical records by logging into www.accessmyrecords.com. Click on the 'My Account' and enter in your AccessID^SM and Password to enter your personal records. You can then add or updating your personal information in the easy to use online forms. When updating your records, you may choose to do it yourself or call our 24/7 customer service toll-free number. You can update your records at any time. We recommend updating your personal information when ever you feel there has been a major change in your medical history, minimum at least once a year.

Is My Information Confidential?

All your information is kept in a secured database, which is protected by unparalleled Internet firewalls and encryption using todays latest technologies. Routine checks are made to ensure the integrity of this system.

What If I Lose My Card Or My Card Is Stolen?

If you lose your card or your card is stolen, the first thing to call the 24hr customer service number or log into our website www.accessmyrecords.com and inform us of the loss using your AccessID^SM, Password and the answer to your secret question. Your AccessID^SM will be changed immediately and you will receive your new card within 5 business days.

Can Anyone Change My Medical Details?

Your medical information cannot be changed using your AccessID^SM code shown on your card, as this is a read-only service. The individual would have to have the AccessID^SM and Password.

Should I Become a Member Even If I Have No Medical Problems?

This website contains more than just medical information. It also contains your personal profile such as personal contacts, insurance policy, phone numbers and agents name, your professional advisors and your legal information. This information can be used in emergency and non-emergency situations.

What If I Am In A Hospital With No Computer Access?

If computer access is not available or when in isolated areas, your medical records can still be accessed by medical personnel through our 24 hour emergency hotline at
800-796-7150. Outside the US , call 954-340-0954 for emergency assistance.

Do I Always Have To Carry The Card With Me?

It is recommended that you carry the AccessID^SM Card and or your AccessID^SM Key Tag at all times. This will assure you the peace of mind that you expect with AccessMyRecords.com^SM. Remember the information on your AccessID^SM Card assist any emergency medical personnel retrieve your life saving medical records.

What Is The Cost Of Membership?

A single AccessID^SM membership is $30.00 annually. Couples are $50.00 annually. Children or additional family members tied to a guardian's account are $20.00 annually.

How Are Members Protected?

Members have the ability to store life saving records online such as; emergency contact information, healthcare providers, physicians, medical conditions, medications, allergies, surgeries, professional advisors, insurance policies, living wills, organ donor information and other legal documents.

What Type Of AccessID^SM Do I Receive?

New members receive three types of AccessID^SM; A wallet size AccessID^SM Membership Card, an AccessID^SM Key Tag and a Window Decal that may be placed on your car window.

How Long Does It Take To Become A Member?

Immediately after joining AccessMyRecords.com^SM online you can print out your temporary AccessID^SM Card including all the benefits of an accessmyrecords.com membership. You now have the ability to add to and update your personal records immediately. You will then receive a permanent AccessID^SM Card, AccessID^SM Key Tag and Window Decal in the mail within the ten business days.
