The AccessIDSM Card contains the name and AccessIDSM code of the member. This card provides emergency service personnel the information needed to retrieve vital medical records and legal documents that will assist them during an emergency situation. Medical personnel may simply log into www.accessmyrecords.com, enter the members name and AccessIDSM code found on the member's AccessIDSM Card or they may call our customer service. They will be provided with a read-only summary of the member medical and legal records. Your medical information is otherwise completely private and secure.
You can enter your personal medical records by logging into www.accessmyrecords.com. Go to the Members Login and enter in your AccessIDSM and Password to enter your personal records. You can then add or updating your personal information in the easy to use online forms. When updating your records, you may choose to do it yourself or call our customer service toll-free number. You can update your records at any time. We recommend updating your personal information whenever you feel there has been a major change in your medical history, minimum at least once a year.
All your information is kept in a secured database, which is protected by unparalleled Internet firewalls and encryption using today's latest technologies. Routine checks are made to ensure the integrity of this system.
If you lose your card or your card is stolen, the first thing to call the customer service number or log into our website www.accessmyrecords.com and inform us of the loss using your AccessIDSM, Password and the answer to your secret question. Your AccessIDSM will be changed immediately and you will receive your new card within 5 business days.
Your medical information cannot be changed using your AccessIDSM code shown on your card, as this is a read-only service. The individual would have to have the AccessIDSM and Password.
This website contains more than just medical information. It also contains your personal profile such as personal contacts, insurance policy, phone numbers and agents name, your professional advisors and your legal information. This information can be used in emergency and non-emergency situations.
If computer access is not available or when in isolated areas, your medical records can still be accessed by medical personnel through our customer service at 800-796-7150, outside the U.S. call 561-443-5508 for emergency assistance.
It is recommended that you carry the AccessIDSM Card and or your AccessIDSM Key Tag at all times. This will assure you the peace of mind that you expect with AccessMyRecords.comSM. Remember the information on your AccessIDSM Card assist any emergency medical personnel retrieve your life saving medical records.
A single AccessIDSM membership is $30.00 annually. Couples are $50.00 annually. Children or additional family members tied to a guardian's account are $20.00 annually.
Members have the ability to store life saving records online such as; emergency contact information, healthcare providers, physicians, medical conditions, medications, allergies, surgeries, professional advisors, insurance policies, living wills, organ donor information and other legal documents.
New members receive three types of AccessIDSM; A wallet size AccessIDSM Membership Card, an AccessIDSM Key Tag and a Window Decal that may be placed on your car window.
Immediately after joining AccessMyRecords.comSM online you can print out your temporary AccessIDSM Card including all the benefits of an accessmyrecords.com membership. You now have the ability to add to and update your personal records immediately. You will then receive a permanent AccessIDSM Card, AccessIDSM Key Tag and Window Decal in the mail within the ten business days.
