We will not be responsible for any original documents that are inadvertently sent to us. They will not be returned to you. After we receive your documents we will upload them into your account, and then destroy the copies.
It is important that you print clearly whenever you send your profile information to us to be added to your account such as any updates to your contacts, doctors, medications, medical condition, allergies and past surgeries. Access My Records, Inc is not responsible for any information that is sent to us which is incorrect. Mailed information will take ten days to process before a member will see it in his or her account.

Be advised that the information contained on the Read-Only Emergency Medical Report has been provided by AccessMyRecords.comSM and created by the member or their authorized representative. or (ii) if the member is unconscious and without an authorized representative and his Emergency Medical Report is accessed, it is understood that the AMR member has acknowledged in the Terms and Conditions when first joined the AMR service; consented to receive an AccessID Card and Key Tag and carries it on his person enabling emergency personnel and or any 3rd party to access the member's Read-Only Emergency Medical Report at any time using the members Access ID code and name.
AMR has not created this information. The AMR member is the owner of this information and has the sole responsibility for it being current, accurate and complete.
