What personal information does our website seek to collect from your child?
When a parent attempts to sign up their child, the parent will be asked to provide his or her name, date of birth, e-mail address, mailing address, day phone number and credit card information, personal healthcare information, as well as to create a password.
We will not collect or retain any personal information from a child under the age of 13 unless you agree that you are the legal parent/guardian of the child you wish to enroll. In addition we will not collect any additional information other than that which will facilitate a child’s medical care in an emergency.

Information about children under the age of 13 whose parents have enrolled online will be used by Emergency Personnel to notify doctors, hospitals, emergency rescue teams, and family members that the child was found carrying an
Access My Records, Inc. AccessIDSM Card or AccessIDSM Key Tag. The child’s previous conditions, medications, and dosages, and any other health issues will be conveyed to the above parties. Personal and health information will also be conveyed to the responder for the purposes of treating the child in emergency situations.
Security procedures in place to protect the loss, misuse, or alteration of information:
We are required by law to take all reasonable and necessary security measures to protect personal information about children. Registration information submitted by a child's parent cannot be modified except by the parent or guardian that enrolled the child. All such information is contained behind secured networks and is only accessible by a limited number of employees and contractors who have special access rights to such systems. All contractors have signed confidentiality agreements with us. All credit card information supplied is transmitted via Secure Socket Layer (SSL) technology and then encrypted into our databases to be used and accessed only as described in this policy.
The Parent or legal guardian who enrolled the child with Access My Records, Inc. may review or modify the information about the child within that child’s electronic medical health record. Before disclosing information about a child, we may request that the parent or guardian provide reasonable information about the parent for identification purposes, such as the parent's name, address, phone number, credit card number, or other information.
How you can opt not to share personal information collected about your child with 3rd parties, if you choose.
Children under the age of 13 will never be asked to consent to disclosure to third parties, and their personal information will not be disclosed to any third parties other than the agencies listed above. A parent's or guardian’s consent to our collection and use of personal information about a child will never be treated as consent to disclose that information to third parties.
If a parent or guardian wishes to terminate a child's membership at any time, this can be accomplished by calling 561-443-5508 and requesting the appropriate form for termination.
